Why I Created It

I'm Dr. Christine Johnson-Nganbou Kamkeu, DNP, AGNP-C, APRN-CNP, a board-certified adult-gerontology nurse practitioner. I've cared for older adults for 14 years in long-term care, med-surg, the ICU, hospice, and primary care, both as a case manager and as a provider. I've written the notes, reviewed the notes, and seen what happens when a chart can't speak for the care behind it. I built this toolkit so clinicians and agencies can catch those gaps while there's still time to fix them.
